The ones in the hospital are so desperate to feel neglected, they won't eat, they won't take medicine, they'll even shit themselves and fight anybody that cleans them up in order to get an infection.
The bariatric patients can also be bad, usually they request food to be prepared in a special way and refuse to eat it otherwise, but at the end of the day if you bring them Jell-O and ice cream they will eat it. The poor patients are so desperate to maintain their self-pitying ideology, they will literally kill themselves rather than changing their self-concept.
