All questions came with comment boxes. 20 and 30 outright stated that you could/should make your own suggestions. 34 and 48 are quite binary, but they still came with comment boxes in case you wanted to give other suggestions such as:

We did not try to imagine every single scenario for the sake of completeness, because that's a lot of work. We assumed that having two separate opportunities to voice your suggestions was sufficient. I hope you put your "Quetzar works with someone else" and "Quetzar brainwashes Northington" suggestions in the comments to the last survey, because otherwise they're not going to be considered - you had your chances.
I see nothing wrong with these questions.
