It is but its extremely unlikely. I'm pretty sure you have better odds of finding a shiny than a 6iv ditto. Any 6iv ditto you see will either be hacked or RNG abused.

In the older games, yes.
With dexnav or SOS chaining you can guarantee 4 perfect IVs, so your odds of getting a perfect Ditto are a mere 1/1024. I think only MM + shiny charm gives better shiny odds than that.
