I have a few questions about this webm
What is this piece of equipment called?
Why did this happen? Did the guy improperly use the equipment and invented his own dumb exercise? Or is this how the thing is supposed to be used but he used improper form or went too heavy?

Why it happened is because torque increases with distance from the pivot point. This exercise already makes zero sense without weight with weight there's a certainty of snap city.

The knee jumped out of its joint.
At this angle there is an incredible torque trying to push the part right to the pivot point down and a small bit left to it up.
You can actually see how the downwards force wins, therefore it suddenly drops since its capsule and inner bands rip. The capsule around the joint is now destroyed, the outer bands probably are alright since they did not snap from pull but it was literally just the downward force that pushed the bone out of the joint until the capsule could not resist anymore.
