Print out image and fill out:
Top Left corner: Put your name in Upper AND Lower Case in proper english grammar spelling. Example John Doe.
Get a Post Office box. Then put the box number up there and the address of the post office.
Change your address to this. From now own just put "Exempt" for Zipcodes, or if needed draw a box around the numbers to "remove it from contracts" using the boxing rule lawyers claim to abide by the "four corners of the document".
Don't want to get a Post Office and change of address? Well I guess you want to still let them know where to pick you up, Private Pyle.
Right hand corner: Leave this section mostly blank, this is where the court clerk will stamp when recieved by them.
In some court cases, if you are filing, you can add the case number/Cause of action number on the bottom line.
1. Fill out your name in upper and lower case. See Public Law 94-241, Article III on the bottom. This is the oath needed.
2. Add a state Republic here. Arizona being the last in 1912. If you are in Alaska or Hawaii, I'd pick any of the earlier states or research this further as Hawaii and Alaska were probably always incorporated.
3. Here put the STATE you are in with ALL CAPS (capitonym name). Example: STATE OF CALIFORNIA
4. Put your name that is found on your birth certificate in ALL CAPS. Example: "JOHN DOE"
5. Put your name in Upper and Lower Case. Example "John Doe"
6. Put your name in Upper and Lower Case again, then the year when your Republic State's Constitution was ratified.
8. Again your name in Upper and Lower case.
Where it says "By:____" don't sign this untill you are in front of the Notary for their stamp with proof of ID.
