Fallout NV saw a lot of the devs from Obsidian reaching out to beta testers and modders when it came time to knuckle deep into the work. A lot of them received meshes, texture file wips and plenty of them talked the story. These would eventually be leaked.
Fallout 4 underwent a very similar leaking process but more of the story was put forth. We knew about the Railroad and Institute a couple of years before release. Something that was less talked about is Bethesdas so called "project seamless" that Todd manages to downplay even when talking about F76.

Unlike with FNV we won't be seeing the outsourcing of a Fallout title potentially ever especially since all the spotlight is on the next Elder Scrolls and the upcoming title Starfield which if my sources are to be believed will drop late next year and will take project seamless into full swing with the only loading screens being jumps across systems.
Another fallout title will take advantage of new technology from both ES6 and Starfield to attempt to blend the best parts of Fallout 4's crafting, base building and gunplay systems with FNV's faction and affiliation systems.
There's not a whole lot of talk even among the Fallout team members or the known affiliates and beta testers so the odds of an upcoming Fallout title in the next 5 years isn't very likely.
It won't suck but it will create a lot of controversy like it did with F4.
One thing I would like to share that comes from 2 former Black Isles Studios location scouts now working for Bethesda is they are now looking into more western US locations for the next entry into the series. They told me to look at F76 and notice the subtle change of the Ranger armor.
