It's a pathogen. The reason infection rates are not 100% is that 1) not everyone has the correct hormone expression 2) certain conditions need to be in place for symptoms to show up (exposure to endocrine disruptors etc). They are however, like in the neighborhood of 40-60% which is why you see such a big spike in degeneracy in the West.
