According to this comic, it seems like their dysphoria is what "tricks" them into thinking they still look like men.
Of course all these trannies are beautiful, natural-looking women. It's just this weird mental-illness called dysphoria that makes them feel otherwise.
Not sure if I'm reading to much into this, but this would be delusion on a completely new level.
