The latter is essentially the presence of psychosis on top of all that. Schizotypal can be managed, and even hidden from others because you are aware that the intrusive thoughts are 100% just that. With Schizophrenia, the lines of reality are blurred and the mind is genuinely deceived. The real question is if Schizotypal can degenerate into Schizophrenia due to the taxation on the mind. Either way, meds are last resort, most people just don't try more logical remedies for their problems first like exercise and not shutting yourself indoors all day with no social interaction. At the end of the day most common mental ailments are a result of built-up stress that people do not make an effort to purge.
