I have a manual KitchenAid one like pic related, and the cutting portion goes along the out because 1) there's less binding, this making it easier to cut, but 2) you can reuse the lid and it's not getting into your food.
I'm sure that just blew some of minds. I think so many people use can openers wrong because with the old school ones that didn't use the wheel, you had to leverage the blade back and forth through the inner lid, so people just assumed the cutting wheel was meant to go there as well?
