put a date in the middle of April
Put the name of a large vaccine clinic or hospital that was doing large clinics (I mean like thousands of doses a day)
On the second row put
Pfizer COVID-19
Lot# EW0176
Put a date exactly 3 weeks after the first date you put
Put the name of the same clinic you put in the row before
On the back put where it says return date the day you put for your second dose and your done.
Before you ask its extremely unlikely anybody would check and even if they did they'd have to sift through thousands of electronic entries and you could always claim that they just didn't enter you in for some reason
