Orthopedy is not my forte but i know a bit about nerve damage
If the ECU tendinitis wasn't there before the surgery there's a high chance they fucked up your ulnar nerve during the surgery (pic related)
Chronic pain has a tendency of, firstly, getting worse with time due to the sensibilization of fibers that lead the electric stimulus from your shoulder/pulse to your brain
The second thing that happens is that, if there's commitment of nerves, there's a thing that happens called "neuropathic inflammation": your nerve secretes pro-inflamatory citokines and whatnot even WITHOUT a lesion
So there's a bunch of things that could affect your pain, and without pinpointing the exact cause and just giving you fucking peptides and tylenol will just make the problem worse in the long run
There's tests you can do and etc
I recommend you go see a pain specialist, and for the love of God don't let them put you on opiates unless the pain is unbearable
Getting a second opinion with an orthopedic surgeon specialized in shoulder surgery regarding medical error and whatnot might also be a good idea
