Of course, i do not think most of the deaths that were carried out by spikes on the knee or the leg were committed directly this way, there are two reasons for this:
First it needs some time to die from blood loss due to these types of murders, therefore they could have screamed for help or

Drugs/toxins are major generalized concepts, we inhale carbon monoxide daily and it is a deadly toxin that is not taken by these measures, a toxin can be in food, in air, installed while asleep for a specific time of death, therfore i stand by this theory, but of course IM NOT saying all the murders were carried out by toxins, im talking about certain ones.
