Mistake 1: overloaded forklift
Mistake 2: the load should be carried as low as possible

Mistake 4: seat belt was not engaged
Mistake 5: it seems there was a small curb, which directly caused the accident. Never drive down the curb, construct the ramp instead (use 2x4, plywood etc)
Mistake 6: forklift driver accelerated while driving down the curb (or a slope). Be extremely careful when negotiating a rough terrain, drive with minimal speed, never accelerate
Mistake 7: 2nd chink (acting as ballast) shouldn't be there, he puts himself in danger and also, barely affects the balance.

What should we learn from this disaster: when faced with emergency situation, remain calm. The most competent worker must immediately devise a plan to extract an injured worker. Other workers must strictly follow his instruction. One person should call 911.
