It's more about prioritizing ports and certain traffic, p2p being throttled down and how download speed doesn't equal latency, all of that. If certain traffic isn't being prioritized a VPN can get around that. If I played LoL on my fiber line I get around 42-50 ms, throw on a VPN located in chicago and I get 29-32 ms, this is a routing issue more than anything else though. But they do shape packet priorities.
tldr: routing everything through your local NSA database takes a while, chud, pay more a month and we'll give you the express lane
