You bill them an invoice. Put something generic like "services rendered" or "consulting fee". Whatever faggy shit you want. The provide you a W-2. You pay the tax man.
Is it just an individual? Have them cut you a check for the amount, pay your taxes on that. Say it was consulting or something. It really doesn't matter. IRS only cares that you pay them taxes on what you made.
