On one hand you could have the Fight Club style where the fictional friend is perceived by the MC as a 100% real person. On the other hand it could be far more abstract or Eldridge horror Song of Saya style. Regardless I think the imaginary lover has to have some perceived form, otherwise it’s just a narrative voice in one’s head and while that might be more realistic it’s less entertaining IMO.
