NPD was initially conceptualized by taking histrionic PD and tacking on grandiosity. Histrionic PD in turn is the female presentation of clinical psychopathy. The male presentation is antisocial PD. So all three of narcissistic/histrionic/antisocial PDs are manifestations of clinical psychopathy. It's not an easy thing to miss. These people are really fucked in the head and everyone around them notices. Usually because they're getting treated like shit. Pic related is the commonly used checklist to identify clinical psychopathy.
