This layer protects the head, and until it naturally dissolves, you can't/shouldn't retract the foreskin. It dissolves as early as 3 and as late as 16, with the average being 10 years old. Forcing the foreskin back before the BPL is dissolved then is called "forced retraction". Basically rips the BPL, causes scar tissue, inflammation, UTIs, and ironically phimosis. Any pediatrician recommending cleaning under the foreskin or diagnosing phimosis before 16 is misdiagnosing and doesn't know the basics. He's only taught to cut it off. If the child can gently retract the foreskin, he should be taught to wash with just water, but he shouldn't force it back.
