Even the poorest ones have an IHS somewhere that gives literal free healthcare to natives. A lot from cities go back for dental work, checkups, screenings, ect. There are some rare old time people who live deep off the grid but they mostly keep to themselves.
It's possible to get out tho and the better ones do. Most that live off rez marry white and have bleached kids. Native hapas look white af, probably because most Americans are mutt.
