Sex work carries with it a sense of deep shame because of how our society perceives it. Because of that shamefulness, the women who do sex work almost always do it because of a history of trauma, and a dysfunctional relationship with their own sexuality.
Actual prostitutes are typically very discreet. Being exposed as prostitutes/escorts can be a very painful experience that jeopardizes women's lives. Pornstars don't have the luxury of discretion - they get recognized in public and can never really escape their pasts. Nearly all men and more than half of women watch porn, so even if we don't talk openly about it because it's a societal taboo (much the same way we don't talk openly about masturbation), the fact is even small scale pornstars are recognizable to huge audiences. The shame of being seen as a whore must be very difficult, plus, as mentioned before, these women have histories of trauma, which indicate mental health issues, possible addiction issues, and loads of other problems.
