I mean, it's not like it's an insanely resource-consuming addition to put in.
They already have the functionality of keeping track if you're non-lethal or not. All they would've had to do is place it in more areas. A couple of fan-made mods do it pretty well and have it stay consistent throughout the whole game, so, it couldn't have been that hard.
And for many of the NPCs you can interact with, they either already have sets of voice-lines recorded for you killing everything, or just random specific things that you have to get lucky or go out of your way to hear. Why couldn't they also have more of NPCs saying "cool" or "ur a pussy" if you didn't kill anything, or went a certain route? It's not as if they'd be having them record a massive amount of extra lines or have it make any real impact on how the game plays out, just a tad bit of extra acknowledgment for non-lethal players would've been nice, because small things like that really make you feel rewarded for your playstyle. It comes off as a cop-out because it's only really seen at the start of the game, has you saying "wow, that's cool" and never shows up ever again. Once you notice that, you really do feel cheated. I don't know if I'll have a non-lethal playthrough ever again, because why not just bash people's heads with a nano-sword while not doing so won't make you feel rewarded in any way?
The practice of putting the most care and detail at the start of a game to keep you hooked on the rest of it isn't an uncommon one, or really even that scummy as one might think, but I feel like this is one thing they could've kept consistent throughout the rest of the game if they just put in the extra bit of effort for it.
Especially since you're incentivized for it at the start of the game. Paul won't stop nagging you about how bad it is to kill people and all, so, you'd figure that it's kind of a big deal.
