If someone is Isekai-ed, this 3D person will perceive their new environment in 3D.
Pretty much all of the over-the-top and crazy actions in Anime, and really all cartoons, are based on a person's internal state of mind, and thus wouldn't be visible to other inhabitants. They would simply be replaced be their mundane equivalent.
