Using foreign phone support that can't speak english. Demanding surguries for life-threatening conditions be pre-approved. Denying the shit out of thousands of stuff that's medically necessary. Lying about how many doctors actually accept their insurance. Charging the maximum price possible when you get a car wreck settlement.
I'm fucked if I get sick while "covered" by any of them, so I might as well drop them next year. I knew our care was bad, but I didn't realize it was royally fucked until I got too old to be on my parents' healthcare.
