As an emergent property, if you do not maintain the continuity then a new instance of your consciousness is nothing but a copy that thinks it is you. The trick to doing it right is by gradual replacement of biological neural matter to electronic neurons or simulated neurons, configured and tuned to act exactly like you would. The transference should be so slow and gradual that the brain itself does not notice the change, with biological neurons only being replaced once they no longer function properly. The technology for this is far from what we're capable of creating.
