From what I've seen of the country's eastern half during the times I've visited/lived in places out there, the only meaningful difference I've noticed is that, aside from enclaves of middle class-and-above suburbanites, basically no one is remotely conscious of their own health, and the supermarket selections tend to reflect this lack of demand for quality food, creating a vicious cycle of growing bodies and narrowing arteries. It's all I can come up with, but even to me, it honestly doesn't seem like a good enough explanation by itself.
