I assume you missed the Fish Market boss (far right side of the market), the Butcher (get killed by the Kidnapper in Lutwidge town near the fountain) or the Oxford University in general.
The perception and attention you need to give in order to get the true ending is fucking MASSIVE so just beat around the bush, get the torches to 10 and enter the door. Whatever happens after the door doesn't really matter. If you get that done I can -gently- string you along towards the true ending.
You'll likely need to grind more in order to get past that Iron Maiden boss. It does have a lot of content afterwards though.
Don't forget covenants are very useful. Get Dodo's covenant if you haven't.
