Depends what kind of attack you use, usually pierce attacks do that kind of thing especially when the enemy is staggered, there's also the mechanic that if push the enemy or the enemy pushes you to the nearest wall you get stagger damage from it too
Floor looks like that cuz that fight was like 5-6 waves so...
