A classic example of something perturbing is assisting to someone having an epileptic seizure. You're used to the human body to work in a certain way and to do certain things and you're in front of a human body that is doing something that *it is not supposed to do*: it's perturbing.
Horror literature and art in general plays a lot with the feeling of perturbing, see zombies (undeads) or puppets/dolls that take life (they're not supposed to be alive, duh) etc
That pic is perturbing because a cat isn't supposed to scream.
