Shit like DBZ and One Piece are already so popular that you can watch spin-off movies for them in regular cinemas in South-East Asia and Latin America. Finding manga and anime-related merchandise at bookstores and toy shops is not that hard in many countries. Even people like Elon Musk and Donald Trump's son occasionally retweet pics of anime girls.
It's by no means obscure, even though there's always gonna be some spots where otaku values (specifically when it comes to stuff like lolicon) will clash against regular people's values.
Guro has really toned down in anime during the last 2 decades-ish, and even though anime with ecchi themes and art are extremely prominent these days, it's quite rare to find uncensored shots of nipples or crotches outside of H-OVA. I wouldn't really say it's normalfag-friendly, because there's still the strong feeling of "otherness" in otaku works that is not present in Disney or Cartoon Network cartoons.
