Remember that it's not just the blast that you have to worry about. All the shit that was near the blast gets irradiated, and if it's light enough to get carried by the wind, it will. So while you may have detonated said bomb in place X, the radioactive dust that got flung into the atmosphere gets carried away by the wind and ends up 2 or 3 countries away (if not even more), poisoning those people too. Ever wonder what's up with the spike in cancers in the last decades?
Not to mention that:
-the nukes of today are a lot more advanced than what they used in Japan (Read about salted bombs. Little boy and fat man ain't shit compared to what we have today.)
-a lot more countries have a lot more nukes
-if you fire a single one, the enemy will retaliate (and there's no guarantee that they'll retaliate with a mini nuke and not a regular one)
It would be a shitshow for both for all sides involved, and for the bystanders.
