1_Neoplastic/Hyperplastic processes: testicular carcinoma, epididymal cribriform hyperplasia….
2_Hematocele/Hydrocele: because when you’re morbidly obese you can press vessels by your own weight and force a 3rd space syndrome trapped by the scrotal fascia (spermatic cord).
3_Infections: there are some viruses that can get in there and force a continuous epididymitis state or even induce tumors.
4_Vessel malformations/torsions.
5_Autoimmune processes.
6_Genetic/molecular pathology stuff: you produce anomalous proteins due to adquired/puntual mutations.
