What they create in Italy is solely for an taste aspect, not specifically for health side of it. Idk why Greece is ranked higher than Portugal but like for example comparing the fish imports from Norway (the cold waters) and from Portugal, the Norway sells much better fish because it's not exposed to hot temperatures, so it tends to be much safer when consumed raw. But that's beyond the point. This poll is always biased because it focuses on price. And the shit Italy produces is heavily focused on a taste and not health side of it. So it'll score higher, but this is mostly because it's much much more expensive than you could rise your self if you live in a secluded hills/mountains, but do not export or know a shit about it, and merely sell it to your country or neighbors. Italy is for this reason heavily overrated. And I'd assume Greece as well compare to Portugal (since it's basically a same mediterranean cousins, based on olive oil and a sea food). I guess they rank higher because their food is valued differently as they care more about tourism. All of this is heavily biased towards a country ability to export it's products to the rich, and organize everything about it.
You know what this means, the actual population of the country that produces this "food", cannot afford it them self's.
