In this instance, you only experience a small portion of the "schizo" or divided mind portion of schizophrenia. The exercise works because for someone with a healthy mind, it is not that difficult to distinguish between the artificial stimuli and external ones, although the process of doing so is still meaningful since such a blatantly artificial stimuli is never experienced so surreally for someone with a healthy mind.
You are describing a severe splitting, and the severity of your delusions also suggests an inability to distinguish between internal and external stimuli, which is blatant psychosis. I hate doctors but if you are not trolling then you are in genuine need of professional assistance, and if you do not seek it your condition will deteriorate to the point where you won't be able to do so on your own terms.
