the family essentially sought out a full-time caretaker and sexual partner for their disabled son which morphed into a career in of itself when the family/couple discovered that you can generate a lot of clicks by piquing the internet's curiosity.
there's probably something going on with the woman to be able to stick around as long as she has (besides the financial security) whether that be a fetish of some sort, perhaps asexual, maybe it's an open relationship, whatever. it's two consenting adults, they can do what they want.
anecdote but i've worked with patients from wealthy areas and i had a man with a traumatic brain injury that required 24/7 care. the family had hired a very attractive woman from some random eastern european country to be the primary caretaker. she couldn't even transfer him from wheelchair to bed on her own but she's meant to be responsible for him which made no sense. i accompanied them to his room to watch her work because i was curious and worried about safety and the male patient became very handsy and touchy (in the capacity that he was able to be) with the woman as soon as they got to his room. she played along, smiling and giggling like it was completely normal which abruptly stopped when she remembered that i was with them. asked if she'd be okay and if she needed any help and she replies with a heavy accent and broken english that she would, all the while this dude is pawing at her with more energy and excitement than i've ever seen him possess. these sorts of arrangements happen, i have no doubt the people in OP's pic have something similar going on (or, at the very least, it started out that way).
