Shanghai and Beijing are quite clean, Hong Kong is obviously extremely clean (if you count it), but nearly everywhere else is filthy. A large part of it is the lack of civic virtue (if I piss on the street or throw garbage on the floor, someone else will clean it for me). Even then, the amount of filth is rather comparable to most large cities in our country.
