If CIA hadn't added "you're a big guy" and given Bane the chance to clarify for whom it "would be extremely painful", the talk would have been over there, no questions asked about who would be feeling the pain.
Bane's "it would be extremely painful" entirely relied on CIA telling him that he's a big guy to set him up for his punch line.
