misskey.io is just an instance of misskey as a software. You can also follow users from mastodon and pleroma instances as well since they all use the same ActivityPub protocol.
However, you do miss some stuff not being on the main instance. Reactions only appear from users that are federated with your instance and some other things like not being able to react with remote custom emojis (for some reason). You can still react with your own custom emojis tho. Also, there's no way to see old posts from before you've federated with a user unless someone else renotes those old posts to appear on your feed (I'll renote old posts on misskey.art if I want to see it on mine)
