Stop watching pornography and jerking off. If you really must jerk off and want to give in to the urge at least tell yourself what you're rewarding yourself for. At least make yourself do something you know you need to do (some chore or exercise) as the reward before you casually just do it out of boredom. At least let your reward system work for you rather than against you.
Next easiest thing to do is cut down on the amount of processed crap you eat. Start aiming for one meal a day where it's not processed slop. Aim to just limit yourself to one or two "treat" days where you allow yourself to eat bad food that has things on the label that are difficult to pronounce
Cut down on alcohol. If you're drinking regularly decide to go sober for a month. Make a conscious decision then if you want to continue drinking, but do it in a structured way (not to get drunk, only socially, etc). Alcohol is a depressant and it's very easy to just get into a routine where you drink out of boredom and then just get used to knocking them back.
Maintain an excercise routine. It doesn't matter what exactly it is, it might be lifting or body weight exercises, going for a run, etc. But aim to get yourself breaking a sweat through exercise at least once a week, ideally twice a week. Getting into a routine is the most important part. Then you can build momentum and push yourself.
Another thing, most people some or even all of the above and then just give up and fall back into old habits after a few weeks or months. And then wonder why they feel like shit again. And will proclaim that the above things "don't work" because their expectations were that it was going to be like a miracle cure that would fix them quickly. It takes time for your brain to re-wire itself and for you to truly get out of a rut.
