1) You upload image + text to a server (there's many, let's call the one you use Akiba)
2) Akiba creates the post details (assigns ID, measures resolution etc)
3) Akiba sends the picture to 4chan's image host (4CDN)
4) 4CDN picks up the image, and saves it as the current time in microseconds (so like 15937392749.jpg)
5) Akiba publishes the post across all 4chan servers, and people see it
Now this is what happened:
1) I upload a post to Akiba, you upload one to Shiki (so, 2 different servers)
2) The post details are created (in which the resolutions of the image differ)
3) Both servers send our image to 4CDN
4) 4CDN first sees your image, and saves it as the current time (say, 1836383749839.jpg)
5) Then, 4CDN se es my image, and tries to save it as the current time (1836383749839.jpg) - however, the current time has not changed, so it simply deletes your picture and replaces it with mine
6) Unknowing about 4CDN's screwup, Akiba and Shiki both publish our posts, linking to the same 4CDN image
It's quite insane, and it almost never ever happens (and when it does, it's usually only a broken thumbnail, not the whole image)
So that's cool, and even cooler when it's two quads
