Sure it did get modified to hell and back but it's not stable, modern versions can barely use 4 cores, the version that FO3 runs is so horrid in fact if it has more than 2 cores it crashes and freezes like hell, performance due to basically no optimisation is horrid. And so on and so forth.
The Engine Fallout 4 is running on was released the same year Fallout was released, way back before Bethesda even had the rights to the series.
If the engine doesn't fuck up for you then honestly I am happy for you, but I need to mod FO3, NV and 4 to hell and back to make it stable. And trust me I know how the fuck that sounds but the engine is a clusterfuck.
