step 1, lose your ego. no matter how much of a loser u seem to b just let it go and admit that you want to improve (<- most important)
step 2, create a daily rhythm in your life. go to sleep before 10 ALWAYS and force yourself to wake up at dawn and go for a walk. if you can do that for a week everyday then GUESS WHAT! YOU GOT YOURSELF A HABIT! Slowly add things onto your morning routine. pushups, stretches, squats, do some running. Checkout this dude on youtube called elliot hulse, he's a philosopher and fitness guru very smart man.
step 3, talk to your family and discuss that you want to improve but that you need their help (which is 100% true). buy healthier foods. Lookup what foods to eat under that hulse guy, he has videos about it.
If you can force yourself to do this for 1 month, i guaruntee youll b a new person op.
after you've done it for a month, hard part is over reach for the stars. you have a foundation.
