if it's the establishing city it can still be considered the setting since that's where all the character backstory and connections are coming from even if the majority of the campaign is running around avernus or whatever the city that gets sucked in is called. presumably there are other people from baldur's gate running around otherwise they wouldn't bother establishing organizations from the city in the first place
