That's different. You should be able to recall the image and see it as a non-substabtial overlay, even with your eyes open. Try closing your eyes and visualizing something new.
If you need a reference point, consider thinking about sound first. Think about the difference between remembering a song and actually hearing it.

Everyone has talked to people who are working it out as they go. It's an approximation of human thought learned from seeing others do it. They are piecing together words from definitional memory in an order to communicate something. People with fully functioning brains are deploying language as symbolic tools to convey fully realized ideas. I agree with this. However, the actual inability to articulate unexpressed thought in language or not being able to use the "mental snipping tool" for audio playback is a terrible downgrade.

I can easily picture it in my mind, in color, from all sides but with open eyes, like I can watch real life and my mind at the same time. But closing my eyes makes them twitch all over the place and while it's easy to see the apple, it doesn't come naturally and puts a strain on the back of my eyes.

Now that I think of it, the only times when I think with closed eyes is when I fall asleep from exhaustion. Usually my vision is like two sheets of paper (one for inside my mind, one for what I actually see) one of top of the other and I can adjust the opacity of each depending on how much and what I need to think, if that makes sense. Usually I keep the bare minimum of "life vision" just so I don't get run over by a car, and spend time in my mind, I like to think.

Am I an NPC? I have an inner monologue or whatever it is called.

Anyone else in this boat? I can see an apple almost perfectly in my head if I choose to, but otherwise "apple" is a conceptual object. When thinking about an apple, you don't have to animate the entire object in your head complete with color. In my mind at least, it compresses the concept of "apple" to a quick outline of it, then transitioning to a dictionary of its properties (approximate size, usual color, taste, etc).

"Third person aerial" is a neat program. I use it to navigate the grocery store sometimes and used it in sports quite a bit.

Makes you wonder what else you can do that you haven't unlocked, doesn't it? How about heads up display graphical overlay for how different things are moving in real time? Great for walking on busy sidewalks, sports, etc. Think about how The Machine saw the world in Person of Interest or even Iron Man. It even has an extra setting to color shade things by speed and mass.

Most of them dream in images
So they have that part of the brain but aren't connected to it when awake
Some also say they can see the physical shape of letters in their mind

Actually! Start like this. Look at something on the corner of your desk and then superimpose that image to the other side. Maybe start with just the outline. If you can get that, you can start trying to superimpose mentally fabricated artifacts.
