Plenty of diseases could start in an even more unsanitary village, but they wouldn't go particularly far. High population density, being in the middle of it all, a time of travel to and fro is exactly what would lead to a large amount of people being exposed to a large variety of things, and plenty of opportunities for whatever mutations would make it ideal. Furthermore Wuhan and Hubei province as a whole isn't exactly the most developed so the fact that it came somewhere with lower than ideal sanitation in many areas isn't of much surprise either.
That's just my pretty basic knowledge on this, what things are actually like there I cannot speak to.
