The blast (assuming it's a quick eruption) would kill most of the north American continent.

The ash clouds would poison the atmosphere and usher in a long, slow winter worldwide. This would cause mass starvation.

The health effects of the food shortage and the failing utilities would make currently inconvenient medical problems become fatal.

Eventually the ash would spread worldwide and cause slow suffocation as your lungs basically fill with cement.
