It could be about Usui's random delusions, so we can see all the girls in any kind of weird shit. Hell, the entire manga could be about a bunch of male students in the school and their own personal delusions for each girl every chapter. We could have Usui's sometimes, some of the other male main characters's delusions of the girls, or even some otaku (fat student) delusions. Imagine one otaku loner student's delusion about making all the girl's his own harem sluts (basically an NTR delusion).
On another note, this delusion chapter is one of my favorites. Well, aside from the one that had Aki's pasties in the garbage can, confirming that it did actually happen. This one has one of the main heroine's and love interests participate in Usui's slutty delusion antics, something Takeda seemed to want to avoid early on into the delusion series (to avoid NTR feels).
