I've done the usual penetration, etc., requests as commissions because, hey, the customer is paying for it and more power to them, but aside from "skill issue", I always had a lot more fun experimenting with clothing, styles, and surroundings. Really, /hdg/ shifted away from that in favor of complex variations on "Hey, how magical can I make the slime ramming into said girl's holes or out of said holes look?" Which takes labor and practice, but it means diverting that from things like fashion or environment. I suppose it boils down to, "How many variations on two faceless dudes splitroasting this huge-titted lookalike can I actually do?" So, skill issue.
And another part is, as you said, a sentimental choice, I actually quite like the characters I made along the way. It is funny running into people who followed me on Pixiv occasionally, or find the page made for me on Aibooru.
