Realistically, as you said, they should change the system by taking into account more factors that are just as important as bust and underbust cm (shape, type, overbust, location of fullest part of breast, ribcage width, e.t.c.) Sometimes certain stores will do this for you. Taking 3 measurements of your boobs, and then having you lay down, sit, and lean to account for all angles.This will create a better fit, but obviously will be more expensive.
So yeah, the system we have now is far from perfect, and as you excellently put, does not accurately support women with massively disproportional bust to underbust ratios or women over a certain cm who have to custom order bras.
However, a large undderbust DOES "offset" a large bust measurement. The rule of the system is large bust cm+large underbust=smaller cup size, large bust cm+small underbust=larger cup size. Nene Fujinoki, who has very sizable, but not the biggest bust (98) is a good example. Her cup size is I, because her frame is so tiny and her underbust is so small. Her breasts are so disproportionately large compared to her midsection.
We are unfortunately bound to this system when we note the cup sizes on the chart. We have to make the chart readable to the masses overhauling the current system and replacing it with our own would not make much sense. Nobody would know what any girl's size would actually be as it pertains to the current system.
