You start in your house, it's a central hub of sorts. You can save, review missions, and other stuff.
When you leave your home, you have to run around collecting small sparkles (how many you have is shown in the top screen, beside a key icon). You use these to unlock missions (which are found all over the town, and they look like little machine things).
Unlocking a mission will reveal a red star in town. Head to it and you either have to do what the mission says, or a scene will play out where you do the mission (it depends on the mission itself).
The entire time, you have to avoid girls (who lower your HP by 1 if they spot you), men (who will chase you and rape you for 3HP if they catch you), men in suits (who will kidnap you and sell you into slavery), policemen (who will arrest you), and cars (who will run you the fuck over).
The game actually gets HARDER the more times you get caught. Getting seen by girls makes more policemen prowl around, for instance.
On top of this, you have 30 days to do 10 missions. You CAN take longer, but enjoy the worst ending. Not to spoil the story, but let's just say you're not doing this for the thrill.

Then, when I realize that was getting what I wanted all along, I'm disappointed that we won't get the magical shenanigans I was hoping for. With the dress, anyway.
